I wanted to see if I could use Adriana Trigiani for 5A. Here's the description of her TV work from her bio:

For #5, part B would the author Alexander McCall Smith work for the last name Smith? Since it is not hyphenated and I'm not sure if McCall is a last name or middle name I thought I would check.

If it's not hyphenated, then Smith is the last name (and of course, it would also work for "author using 3 names.")

That would be fine...as long as it's set in a location you have visited but have never lived in.

Would this book work for 3A-Canadian author? She is American but lives/works part time in Canada as stated in her bio.
