Usually means the book was first published under one version of the author's name, and later the other.

If a cover image has a particular version of a name on it then that is the name the book is listed under. If the author has a better known name, or a second version of a name ie. "Dean R. Koontz" and "Dean Koontz" then there is a reason for it and we abide by it.
The books I referred to as “clearly Dean R. Koontz titles" are the books which clearly have "Dean R. Koontz" plastered all over the cover and which should not be changed to another name. The Star Quest edition should have been "Dean R." and I've changed it.
In this case the easiest fix for the editions you have changed is for you to change them back (if they need changing) because you know what you have changed on which editions. If you know of someone else making the same sort of errors on Koontz books then you could send them a link to this thread.

Just to be clear since all editions have Dean R. Koontz on the cover, that should be the name listed right?
If so I will fix this one book and add data to the Dean R. Koontz profile, including a note. (I will have to leave any other Dean R. Koontz books for another librarian or another day.)
Ugh. Yes, all editions with the middle initial should have that profile. (IIRC, he's also one of the authors who publishes different types of books under different variants of his name.)
