I publish mine using M'soft Word 2007 and KDP. Make your chapter titles be format Heading 1. Add a page near the beginning of you book titled Table of Contents and put the cursor underneath it. Then in Word under References tab choose Table of Contents and Insert Table of Contents then pick Heading 1 only with no page numbers. Then put the cursor at the top by the Table of Contents, go to Insert and choose Bookmarks and then type in toc. That should do it.

I "finished" my story a month ago, but all this formatting and other tweaking for publication is taking longer than I thought. Just a matter of learning the process, but there's a lot I didn't even think about.

I d/l Calibre, a free app that a seems especially handy for epublishing. The TOC function in Calibre did a great job of constructing my TOC. It was easy for me to use.

1) Do I have to do the Table of Contents index for my book or is this something generated by the distributor - KDP, Smashwords, etc. - when I upload the book?
I looked at some blog posts about formatting but didn't see anything that was clear about this.
2) If I put a URL in my book, e.g. a link to my blog, does it get translated okay?
