What's the policy for primary author name on graphic novel adaptations of novels? The original author or the adaptor?

For example, The Graveyard Book Volume 1 & 2 have P. Craig Russell (who adapted the book) as the primary author, but The Graveyard Book has Neil Gaiman, the original author.

The Adapter should be the primary author, and they should be under that profile and not combined with the original.

Make sure to leave a really specific librarian note, and expect to have to go uncombine them and fix the attribution on a regular basis too (particularly with an author as popular as Gaiman) :)
