Whirligig After MC Brent Bishop is embarrassed at a party. He decides to kill himself during the drive home by closing his eyes and taking his hands off of the wheel. He gets into a car accident killing the other driver. (Her mother even thought about killing herself after daughter death)

My Sister's Keeper Kate told Anna to sue for medical emancipation because Kate did not want Anna to have to donate her kidney believing she will die anyway. Their mother Sara will not let her go, forces both of her daughters though painful medical procedures. Anna dies in a car crash after the trail. Kate gets her kidney anyway and goes into remission.

Before I Fall Tried of being bullied Juliet tries to kill herself by jumping in front of a car MC Samantha saves her by pushing her out the way, but is hit and killed instead.

It doesn't exactly fit the description but Ethan Frome has a suicide pact that doesn't go according to plan.

On a Pale Horse It's fantasy so may not be what you're looking for, main character tries to kill himself but instead accidentally kills the personification of Death and has to take over the job.
