I noticed a librarian that I've seen making changes against the policy in the past made another change that I suspect is against the policy (replacing ARC cover and title, with the published one), but I can't be 100% sure of it, as the author requested a change here, but didn't confirm if ARCs were distributed with previous cover.

What is the correct course of action in this case?
I don't think I should flag if I only suspect the change is against policy.
Is there any way to check if the cover was distributed with ARCs?

Should I reverse the change and wait for author to reply?

That seems reasonable. However, to answer the general question, it's fine to flag things you are unsure about, as long as you state that in your comment on the flag.
