#890 combined, the typo is on the cover (also on title page of original 1815 edition)

#891 I debated but decided reference was to journal in which article, then published as book, appeared. Is page count for entire journal?

I don't think so.

Insulin Therapy is 270 pages
Insulin Therapy, an Issue of Endocrinology and Metabolism Clinics is 960 pages

It might be worth making a project out of it (outside of this thread), as there are also issues with the author John L. Leahy vs Jack L. Leahy.
