As far as we can tell this chapbook has just a blank page at the front. Is it then acceptable to use the frontispiece as cover image?

Personally, I wouldn't do it. Per the manual: "Cover images uploaded should only be an image of the actual front cover"
I think it's a similar case as old hardcover editions using generic covers without any kind of text (e.g. https://www.goodreads.com/book/show/1...)
