Unfortunately, if librarians are combining from the author page, they often don't even see the book descriptions. I would still advise notes in the description field as it will prevent some confusion.
I had once combined some "Complete" books and a librarian messaged me to tell me they weren't the same book and another librarian messaged me once when I'd made another combining mistake. I didn't take those messages as attacks but appreciated the information, and I now don't make those same mistakes. Messaging the librarian that incorrectly combined them would help that librarian not repeat the mistake but does not really address making the situation clear to all librarians.
Sometimes I wish we could put additional clarifying information in the title, but I guess that is not the way to handle this. I cannot think of a way to prevent this from ever happening again. It's possible another librarian will have a creative solution for this because it's not an isolated incident.
