What is the difference when it says a reader "added" the book instead of put on their 'to read' list?

Added means you read it. To read means you intend to read it. It you look on the left side of your screen (you may have to scroll down), it will list your bookshelves. This will show the numbers for your lists. If you click on the label, to-read for example, it will take you to the list of books you put on it. Play around with these links. You'll get the hang of it in no time.
