In 'Wintergirls', why are they called Wintergirls instead of ghosts?

Laurie Halse Anderson Because Wintergirls are not dead... not yet. But they are not alive, either. They are so tangled up in the illness of their eating disorder that thy are frozen between life and death.

In the book Cassie is truly a ghost, because she is really dead.
